I /We observe and agree with the following rules made by the organising committee :

本人谨此声明遵从下列条款，并无异议：

1.The personal data provided in this form will be used for processing your application or admission, and for registration, academic and administrative communication. All photo and related information will be placed at the website ofAEDC & HKYDA for publicity.                         

个人资料声明：同意亚洲精英舞蹈大赛及香港青少年舞蹈总会收集报名填写的个人资料，目的乃协助大会联络参赛单位及作日后举办活动联络之用。活动有关相片及相关资料将有机会刊登于香港青少年舞蹈总会及HKATOD网页或其他宣传品内，作宣传用途。

2.For the contestants under 18, their parents or guardians have to sign the consent form for performance and competition.

未满18岁的参赛者，其家长或监护人同意及必须签署《监护人演出或参赛同意函》（此函会根据赛区当地政策需求调整）

3.The Organizing Committee have reserved the right to reject your application.

大会有权随时拒绝参赛队伍的申请。

4.The contestants or groups will be disqualified should there be false information has been filled in the entry forms.

参赛单位在报名表内所提供的资料必须正确无误，如参赛单位不符合参赛资料或虚报资料，大会有权取消参赛单位的参赛资格。

5.If I/We do not follow the Competition Schedule, no show is non-refundable.

参赛单位必须按大会指定的比赛时间出席比赛，如参赛单位未能出席，将当作弃权论，报名费用恕不退还。

6.No appeal for this competition. The decision of the judges is final.

参赛单位必须服从大会评审团之评审决定，不得异议。

7.I/We should be responsible for our own safety. The Organizer is not liable for any lost and casualties.

参赛单位于比赛中发生的任何损失或伤亡，组委会无需负上任何责任及赔偿。

8.No refund regardless of any circumstances

同意所有参赛费用，不接受任何理由的退款申请。

9. I consent that if registered, I will conform to the statutes and regulations of the organising committee of the competition.

本人明白及愿意遵守章程及上述细则，并服从大会之各项安排。
